The Requirements to Join PCNOK and How to Get Started

Patients must meet eligibility requirements in order to receive PCNOK services. These requirements include residency in Oklahoma, enrollment in a qualifying health insurance plan, and annual household income at or below 133% of the US Federal Poverty Level. Patients need a primary care physician and a current driver’s licence or state ID from Oklahoma. It’s simple and cost-free to join PCNOK. A completed application form and proof of residency are required of all applicants. After being accepted, members have greater access to healthcare, education, and advocacy.
